CaredFor Launches Post-Discharge Measurement-Based Care Platform For Addiction Treatment Provider Organizations

CaredFor, a consumer engagement and outcomes application for addiction treatment provider organizations, announced the launch of a post-discharge Measurement-Based Care (MBC) platform.
According to the company, the platform is intended to track consumer outcomes from pre-admission through post-discharge recovery. Consumers use a mobile application to complete standardized assessments and report measures including cravings, sleep, mood, and anxiety. The platform also includes a contingency management feature that allows consumers to earn points tied to assessment completion.
Consumer assessment data is available to health care professionals through integrations with electronic health record (EHR) systems, dashboards, and reporting tools. The platform also includes optional AI-based risk alerts intended to identify consumers who may require additional follow-up or intervention.
According to the announcement, provider organizations can use platform data for payer reporting, contracting discussions, prior authorization support, and regulatory reporting activities. Additional features include cohort benchmarking dashboards and outcomes reporting tools.
CaredFor reported that the platform is currently used by organizations including Hazelden Betty Ford, American Addiction Centers, and Newport Healthcare. The company stated that implementation is currently available, with clinical deployment expected within 90 days.
CaredFor is a product of ContinuumCloud, which also provides EHR and human capital management platforms for behavioral health and human service organizations. ContinuumCloud is backed by Battery Ventures.
